My Website hacked

Hi. First sorry for my english.

I have a website (actually a forum comunity) in http://www.manganimecanarias.net. Today someone wrote in my root directory an index.htm and deleted my index.php (which is a redirection to /foro/portal.php...) with arabian letters and some weirds icons. The all /foro folder is intact. I was lucky this time...

Some time ago other guy changed my domain's redirection to similar website, but in italian...I fixed it writing a index.php with the redirection that now is hacked...

My question is...how do they it? and how can I prevent this? Are there bugs in the server? I am using a phpbb plus2 forum, but I don't think that it matters because the forum is updated...

Should I contact with CEO?

Do you have any other scripts you are using or installed but not using? That are not updated? That is the most likely source.

The same thing happened to me about a year ago. By some Turks if I remember correctly.

I just cleaned up all my useless crap in my account and it never happened again.
