Help with Web Form Designer

I've created a contact form with Web Form Designer. All looks good but I am stuck with getting the right command so that someone can contact me with the form. The set up box asks "I am using a script on my server or another source" Then it has a box to enter in the script, that's where I am stuck.
Any help would be appreciated.
